November 21, 2011 — Patients with diabetes who have experienced a heart attack may get protection from future ventricular arrhythmias and fatal heart attacks by consuming daily recommended doses of omega-3 fatty acids, researchers report in the December issue of Diabetes Care.

The researchers studied the records of 1014 people with diabetes aged 60 to 80 years who had had a heart attack within the previous 10 years. They randomly assigned the patients to 1 of 4 groups: people who received margarines containing eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) at 400 mg/d, patients who received margarines containing α-linolenic acid l (ALA) at 2 g/d, patients who received margarines with all 3 fatty acids combined, and patients who received margarines with placebo.

During a 40-month follow-up period, the group that received the combination of all 3 fatty acids experienced an 84% lower incidence of ventricular arrhythmia-related events than the group receiving placebo. That group also experienced a 72% lower incidence of combined arrhythmia and fatal heart attack.

"While more research is needed to definitively determine the role of these fatty acids in protecting people from ventricular arrhythmias, they seem to provide a benefit to the heart attack patients who also had diabetes," lead researcher Daan Kromhout, MPH, PhD, a professor of human nutrition at Wageningen University in the Netherlands, said in a statement. "This is the first study that showed a significant protective effect of Omega-3 fatty acids in high-risk patients with diabetes who were on state-of-the-art drug treatment for their heart attack."

This study was a secondary analysis of the Alpha Omega Trial, a multicenter study involving 32 hospitals in the Netherlands. Previous reports from this trial found no protective link between major cardiovascular events and omega-3 fatty acids for the high-risk patients.

"The key issue to think about is that all of the science — observational science and prior trials — really points to benefits for heart disease death, not total heart disease at low doses [of omega-3 fatty acids]," Dariush Mozaffarian, MD, DrPH, associate professor of epidemiology at the Harvard School of Public Health and associate professor of medicine at Harvard Medical School, Boston, Massachusetts, and an expert on nutrition and prevention of cardiovascular and metabolic diseases, told Medscape Medical News. "That's why this analysis is important, because it looked at a population of diabetes and heart attacks at the very high risk for heart disease."

Dr. Mozaffarian continued: "The strength of it is that they're looking at the right outcome: heart disease death in a high-risk group. The limitation is this is not the original trial, this is a subgroup analysis. It supports the idea that omega-3s are beneficial in people at higher risk for heart disease death, but we would ideally see such results from a larger trial." Larger trials have involved 11,000 and 18,000 participants, he said.

The authors write that a limitation of this study could be the small number of patients who experienced ventricular arrhythmias or died; 29 patients developed arrhythmias, and 50 died of myocardial infarction or other vascular diseases. However, the authors write, the study still showed a significant protective effect for the patients who consumed the combined supplements of EPA, DHA, and ALA. In this, and in the main Alpha Omega Trial, almost all patients were taking antithrombotic prescription drugs, antihypertensive drugs, and statins.

Sources of omega-3 fatty acids include fish and plant oils, walnuts, and supplements.

"At low doses, such as what were studied in this trial, the major established benefits of omega-3s are improved brain function in babies of mothers who are consuming omega-3s and reduced risk of heart disease death," Dr. Mozaffarian said. "Personally, as a cardiologist, I recommend fish consumption as the first, primary, and best approach to getting the omega-3s. Fish has other elements that are good for health, and it's really hard for people to take a pill every day."

The study was funded by the Netherlands Heart Foundation, the US National Institutes of Health (NIH), and Unilever Research and Development. Dr. Kromhout has received grants from the Netherlands Heart Foundation, NIH and Unilever Research and Development. One author has received a grant from the Alpro Foundation, and another author is an employee of Unilever, which markets food products enriched with omega-3 fatty acids. The other study authors have disclosed no other relevant financial relationships. Dr. Mozaffarian has received grants from Pronova ***a, GlaxoSmithKline, and Sigma Tau Pharmaceuticals. He has also received National Institutes of Health funding for an investigator-initiated trial of fish oil to prevent postoperative atrial fibrillation.
